By booking an appointment with Lansdowne Podiatry, you acknowledge and agree to the following:

1. Credit Card on File

• I authorize Lansdowne Podiatry to securely maintain my credit card on file through Stripe, a third-party payment processor.
• I understand that Stripe uses industry-standard encryption and security protocols to protect my financial information.
• Lansdowne Podiatry does not store my credit card information directly; it is securely stored and encrypted by Stripe.

2. Missed Appointments & Non-Covered Services

• I understand that if I miss, cancel, or reschedule an appointment without at least 24 hours’ notice, my card on file will be charged a $85 no-show/late cancellation fee.
• For services not covered by insurance (including but not limited to laser treatments, medical pedicures, and KeryFlex nail restoration), I agree that if I miss the appointment, I will be charged the full amount of the scheduled service.

3. Authorized Charges

• I authorize Lansdowne Podiatry to charge my card for the following:
• $85 fee for missed or late-canceled appointments.
• Agreed-upon non-covered services scheduled at my request.
• I understand that no other charges will be made to my card without my prior authorization.

4. Card Removal

• I may request removal of my credit card from file at any time by providing written notice to Lansdowne Podiatry.
• I understand that if I remove my card, I may be required to provide a new form of payment prior to scheduling or receiving services.

5. Agreement

By checking the checkbox on the checkout page, I acknowledge that I have read, understood, and agree to the terms of this Credit Card Authorization & Appointment Policy Agreement.

I agree and authorize Lansdowne Podiatry to securely store my credit card information and charge it in accordance with this policy.
